How to Apply


First, WHEN to apply ...

The preferred application deadline is January 31, but applications will be accepted until all the available "slots" are filled.
However, it may be to your advantage to apply early because of the required tax forms. If the application for summer 2009 is postmarked before December 31, we can accept the tax forms covering 2007, which were due the previous April. If the application is postmarked after December 31, send the 2007 tax info now, but we will also need to get the 2008 taxes when they are available. this way we will have "current" information on file. DO NOT DELAY sending your application because you are waiting for this year's taxes to be figured. Send the 2007 taxes and your application immediately.

 

Then, HOW to apply ...

There are two (2) ways for you to apply for the Upward Bound Math & Science program.

You may obtain and complete the UBMS Application form

A.   from your high school counselor (or math or science teacher), or

B.    print out the .pdf formatted UBMS application.

In each case you need to "snail mail" the completed application to the UBMS Center.

(Each of these methods also requires submission of (a) your school transcript and
(b) a signed verification of family income.)



A completed application consists of 8 pages plus the transcript
(with attendance and achievement test results), and the income verification. Those 8 pages are:

  • 3 pages of your information,
  • 1 page of essays, and
  • 4 pages total of teacher recommendations.

(The teacher recommendations, transcript, and income information may be submitted either with your application or separately.)

This application must be completed in its entirety for us to fully consider you for participation in the UB Math and Science program. We need all of the requested data in order to assess your academic eligibility and consider all applications in a fair and consistent manner. All of the information required to complete the application is easily obtained.

  • See your school counselor, or Math or Science teacher, and request a copy of the application.
    (It may be copied as many times as necessary or desired.)

  OR

1.     Verify that you have Adobe Reader© installed on your computer.
(A free copy is available in either Mac- or IBM-compatible format from the Adobe website.)

2.     Select "View & Print the Application form" below.

© will open and load the Application form. You may then use the scroll bars to browse through the Application form or you may print the Application by selecting the printer icon or choosing "Print..." from the File menu.

Regardless of how you apply, remember that preference is given to
     * complete applications
     * received on or before the stated Jan. 31 deadline.
